Thank you for choosing HER Wellness Health Center for your healthcare needs.  This page was created to familiarize patients with our office policies.  If you have questions about anything, we encourage you to contact our office and we’ll be happy help you.

REGISTRATION

Upon arrival to our office, you will be asked to complete patient registration forms so that we may establish a record for you at our office.  We recommend that you arrive approximately 15 minutes prior to your scheduled appointment to allow time for forms completion.

Please remember to bring the following items with you on your first visit:

  • A COMPLETE LISTING OF EACH OF ANY MEDICATIONS YOU ARE CURRENTLY TAKING, INCLUDING ANY OVER THE COUNTER MEDICATIONS
  • YOUR INSURANCE CARD IF YOU HAVE MEDICAL INSURANCE

APPOINTMENTS

We see patients by appointment.  Contact our office to schedule an appointment any time during regular office hours.  If you find that you are unable to keep your scheduled appointment, please contact us within 48 hours of your appointment so that we may offer it to someone else in need.

FINANCIAL / INSURANCE

We accept most major health insurance plans.  If we are participating with your health insurance plan, we will file your insurance for you.  Please be prepared to pay any co-payment, deductible or non-covered services on the day of your appointment.  For your convenience, we gladly accept cash, checks, debit and credit cards.

PRESCRIPTION REFILLS

We ask that you allow at least 48 hours notice for prescription refills.  This gives the doctor time to review your medical chart to determine if a follow-up appointment is needed or if a refill can be called in to the pharmacy.  When calling, please provide us with the following information:

  • NAME OF YOUR PHARMACY
  • PHARMACY PHONE NUMBER
  • NAME AND DOSAGE OF YOUR MEDICATION

Please understand that, as a general rule, we do not phone in prescriptions after hours or on weekends.
